I appreciate all the hard work you’ve done and will do to keep our members healthy. As the weather shifts, flu season comes with it, and for people with chronic health issues, catching the virus can become something much more serious than a short illness.
The flu shot is the strongest tool we have, and members are far more likely to follow through on getting one when they hear it directly from you. Please keep having those conversations, whether it’s during an office visit or a quick phone call. | | | |
And while we continue to push vaccination, the basics shouldn’t be overlooked. Washing hands, covering coughs, staying home when sick—small things that add up, especially in group settings and family homes.
If a member is unsure where to get their shot, please send them our way and we’ll help them find an option that works. Refer members to their care manager for assistance. | |

CDNY is running the 2025 Member & Family Satisfaction Survey now through November. The survey is voluntary and anonymous, and has no impact on service delivery. The results simply inform our ongoing quality improvement.
We’ve again engaged Research & Marketing Strategies (RMS), an independent research firm, to conduct the survey amongst 2,500 randomly selected members.
How Outreach Works - Completed by members and their family/guardian/support/advocate as needed.
- Phone survey (English and Spanish options) from a (518) area code.
- Email survey for members whose preferred language is Mandarin.
- Mail survey for members whose preferred language is Yiddish.
Your support in nudging members to complete the survey is greatly appreciated. Participation is optional but highly encouraged to help us better understand member experiences and improve the supports and services we offer.

We’re inviting providers across our community to take part in our new IDD Care Compact — an easy, non-binding agreement that demonstrates your commitment to serving the IDD population and partnering with our care teams.
By reviewing and signing the Care Compact, you’ll help us: - Expand access to quality care for people with IDD
- Ensure your practice is considered for future referrals
- Improve connections between providers, care managers and members
Once you’ve signed on, we’ll include your practice in our network, share your info with our team for referrals and offer opportunities to spotlight your services to the people we support.

Discharging a CDNY Member? Follow These Steps
- Contact the CDNY Care Manager as soon as a member is admitted, and again at discharge. Early communication helps ensure a smooth transition.
- Share Required Documentation, including the full discharge summary, within two days of discharge. Fax to (518) 261-1621.
- Use Member Consent. All CDNY members have given consent via the DOH 5055 or 5201 form to share health information. The Care Manager can provide proof if your team needs it.
